Medical Coder, Home Care Department: Home Care Schedule: Full Time Shift: Day Hours: Job Details: - 1-3 years
JOB PURPOSE: With limited supervision audit cases in homecare and hospice to ensure accuracy of diagnostic coding and compliance with State and Federal regulations and adherence with all policies. JOB RESPONSIBILITIES: Chart reviews for homecare and hospice to ensure accurate coding, quality is documented and compliance is met. Audit admissions to ensure physician orders are accurate and complete. Ensure the case manager visits, PT, OT, SP, HHA, pastoral care and volunteer are ordered and scheduled accurately. Develop a plan and follow up for all disciplines as needed when issues are identified. Work in conjunction with the RN Educator and RN Team Managers to develop comprehensive education plans, corrective action plans, etc. QUALIFICATIONS: High school AA/Bachelors degree, preferred RN or LPN licensure, preferred Certified Coder Experience in homecare, preferred Demonstrated organizational and problem solving skills Excellent knowledge and understanding of medical terminology, home health and hospice regulations
Primary Job Functions Reviews documentation to ensure the correct diagnosis was chosen for the OASIS document, physician orders Verify the diagnosis based on the medical information received Reviews physician orders to ensure compliance with skilled nursing, PT, OT SP , HHA, volunteers and pastoral care Develops action plans to correct issues identified in the initial audit Reports trends and identified issues to Assistant Director Other duties as assigned
